DAY SERVICES

Day Habilitation Services Manager- Hyannis

Full Time  40 hours

Provides support and assistance to program participants in the areas of independent living, self management within the community, social and independent living skills development and affective and behavioral management.  Communicates with families and providers, as required.  Assists program participants in making and expressing choices based on assessments and other service need indicators. Creates goals and objectives. Plans program activities and supervises implementation of services created by interdisciplinary team (IDT).  Assists in the development and expansion of opportunities for community integration.

Bachelor’s degree preferred.  Minimum 1 year prior experience in a community health setting.  Ability and willingness to provide hands-on personal care.  Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.  Valid driver’s license in the state of Massachusetts required.  Successful completion of all state and Agency mandated trainings required upon hire.

Healthcare Supervisor (RN)- Eastham/Hyannis

Per Diem

Provides direct care and training in relevant health topics for staff and clients, as needed.  Coordinates medical services with each participant’s primary physician or medical clinic.  Oversees all health care services provided to program participants.  Participates in the ongoing interdisciplinary evaluation and review of each participant’s needs and status.  Obtains reports and approval of treatment plans from primary physicians.  Documents recommendation in participants’ records resulting from periodic reviews.  Advises program management and staff of any medical problems that may hinder a participant’s progress or that may be negatively impacted by program activities.  Completes, signs and/or reviews monthly progress notes in accordance with regulations established by the Commonwealth of Massachusetts-Executive Office of Health and Human Services (EOHHS).

Registered Nurse licensed by the Massachusetts Board of Registration in Nursing.  Minimum 1 year prior experience in a community health setting.  Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.Valid driver’s license in the state of Massachusetts required.  Successful completion of all state and Agency mandated trainings required upon hire.  Successful completion of Medication Administration Training certification required upon hire.  Must be compassionate and patient.

Case Manager – Individual Supports

Provides instruction, supports and positive role modeling to individuals with varying developmental disabilities and/or acquired brain injury living in their own homes. Encourages the development of life, social, and recreational skills, as well as, community access and healthcare. Provides direct, active and constructive interactions with individuals in cooperation with staff, team members, clinical consultants, community members, families and managers.

High school diploma.Education in psychology, social work, special education, or other related field preferred.Prior human services or relevant life experience preferred.Valid driver’s license in the state of Massachusetts.Successful completion of all state and Agency mandated trainings.Successful completion of Medication Administration Training certification.

Full Time $15/hr plus $1/hr weekend differential

The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access.  Provides direct, active, and constructive interactions with individuals in collaboration with colleagues, supervisors, clinical staff, community members, and families. DSP-2s are typically assigned to residential homes involving more complex and difficult cases including, but not limited to, medically frail or elderly residents, individuals with acquired brain injury, individuals with developmental or intellectual disability, or individuals with other related clinical or behavioral diagnoses.

High school diploma or equivalent required.  Associates degree or additional education in the field (s) of psychology, social work, nursing, or special education preferred.  1 year of direct care, CNA, or human services experience preferred.  Ability and willingness to provide hands-on personal care.  Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.  Valid driver’s license in the state of Massachusetts required.

Residential Registered Nurse – Hyannis

Part  Time 20 hours Tuesday – Friday

In conjunction with residential management, provides a comprehensive system of residential services creating a safe and therapeutic environment that fosters the development of human dignity and focuses on personal well- being.

Registered Nursing License. Minimum 2 years prior experience working in a medical setting as a registered nurse. Valid driver’s license in the state of Massachusetts.Demonstrated experience in community health nursing, visiting nursing and/or nursing specific to individuals with unique developmental or traumatic brain injury challenges. Successful completion of all state and Agency mandated trainings. Ability to complete state mandated certification program to teach coursework developed by the Department of Public Health (DPH) Medication Administration Program. Ability to travel to residential homes throughout Cape Cod to provide medical clinical assessments. Ability to provide hands-on personal care.

SHARED LIVING 

 Community Health Nurse 

Full Time 40 hours

As a member of the nursing and care management team, provides nursing services to facilitate the screening of new foster homes and participants; assists with matching and placement services and maintains services to participants, as needed.

Registered Nurse licensed by the Massachusetts Board of Registration in Nursing. Minimum 2 years prior nursing experience in the direct care of the elderly or disabled adults.Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.Valid driver’s license in the state of Massachusetts required.Successful completion of all state and Agency mandated trainings required upon hire.Successful completion of Medication Administration Training certification required upon hire.Must be compassionate and patient.

Provides safe and timely transportation to individuals with disabilities between their residences and day sites.

High school diploma. Prior experience providing transportation services for individuals with disabilities preferred. Valid driver’s license in the state of Massachusetts. Documented physical indicating fitness for job.  Good driving records.  Ability to follow directions and read maps.
