Executive Assistant – Hyannis
Part Time with possibility of Full Time in the future

Provides high level administrative support by preparing reports, handling information requests, and performing clerical functions including preparing correspondence, receiving visitors, arranging conference calls, and scheduling meetings.  Supervises reception staff.

High school diploma. Associates degree or equivalent experience preferred. 2-4 years of administrative assistant experience.Valid driver’s license in the state of Massachusetts.Excellent computer skills using word processing, email and spreadsheet software applications.Strong communication and customer service skills.Ability to adapt to changes and meet deadlines.

DAY SERVICES 

Day Habilitation Aide –  Eastham 
Part Time 9 am-2 pm M-F

Provides support and assistance to program participants in the areas of independent living, self management within the community, social and independent living skills development and affective and behavioral management.  Communicates with families and providers, as required. Bachelor’s degree preferred. Minimum 1 year prior experience in a community health setting. Ability and willingness to provide hands-on personal care. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required. Successful completion of all state and Agency mandated trainings required upon hire.

Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed by the Massachusetts Board of Registration in Nursing. Minimum 1 year prior experience in a community health setting. Successful completion of all state and Agency mandated trainings required upon hire. Successful completion of Medication Administration Training certification required upon hire. Must be compassionate and patient.

Full Time

Within assigned residence, implements a comprehensive system of highly personalized residential supports that creates a safe and therapeutic environment for individuals served.

Bachelor’s degree preferred. Minimum 3 years prior experience supporting individuals with intellectual and developmental disabilities, traumatic brain injury, or other unique challenges in a community-based setting. Prior supervisory experience preferred. Ability and willingness to provide hands-on personal care. Direct Support Professional II – Various Locations

Full Time/Part Time/Relief  $15/hr plus $1/hr weekend differential

The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access.  Provides direct, active, and constructive interactions with individuals in collaboration with colleagues, supervisors, clinical staff, community members, and families. DSP-2s are typically assigned to residential homes involving more complex and difficult cases including, but not limited to, medically frail or elderly residents, individuals with acquired brain injury, individuals with developmental or intellectual disability, or individuals with other related clinical or behavioral diagnoses.

High school diploma or equivalent required.  Associates degree or additional education in the field (s) of psychology, social work, nursing, or special education preferred.  1 year of direct care, CNA, or human services experience preferred.  Ability and willingness to provide hands-on personal care.  Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.  Valid driver’s license in the state of Massachusetts required.

DSPs are expected to encourage development of life, social and recreational skills, community and health care access.  Provides direct, active, and constructive interactions with individuals in collaboration with colleagues, supervisors, clinical staff, community members, and families.  The DSP-3 will float across assigned Agency homes offering varying programs and services.

High school diploma or equivalent required.  Associates degree or additional education in the field (s) of psychology, social work, nursing, or special education preferred.  1 year of direct care, CNA, or human services experience preferred.  Ability and willingness to provide hands-on personal care.  Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.  Valid driver’s license in the state of Massachusetts required.

Part Time 30 Hours

Provides instruction, supports and positive role modeling to individuals with varying developmental disabilities and/or acquired brain injury living in their own homes. Van Drivers-Falmouth & Mid Cape

Part Time 20 hours and Relief-split shifts

Provides safe and timely transportation to individuals with disabilities between their residences and day sites.

High school diploma. Prior experience providing transportation services for individuals with disabilities preferred. Valid driver’s license in the state of Massachusetts. Documented physical indicating fitness for job.  Good driving records.  Ability to follow directions and read maps.

    VOCATIONAL SERVICES

Supported Employment Coordinator  –  Hyannis

Full Time 

The Supported Employment Coordinator will be responsible for performing professional work in program development and implementation for the Community Based Day Supports and Supported Employment programs. This will include consulting with a team to ensure specialized, person-entered job placements for individuals with intellectual and/or developmental disabilities. Specific responsibilities also include supervision of assigned vocational staff, evaluating individuals’ work capacities, consulting on assistive technology services that will facilitate successful community employment options, overseeing the administration of assessments to determine vocational aptitudes, interests and abilities, and monitoring all employment placements.

Master’s degree in rehabilitation counseling, vocational evaluation, vocational rehabilitation: OR a master’s degree in any subject and one year of professional experience in counseling, job placement, vocational evaluation or rehabilitation. Minimum 5 years’ prior experience working with adults with disabilities including conducting assessments, performing community outreach and managing job placements. Valid driver’s license in the state of Massachusetts. Ability to teach and model occupational tasks.
