Direct Support Professional

Full Time & Part Time

The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access.

High school diploma or equivalent required. 1 year of direct care, CNA, or human services experience preferred. Ability and willingness to provide hands-on personal care. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required.

Direct Support Professional, Individual Supports

Full Time & Part Time

The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access. Provides direct, active, and constructive interactions with individuals in collaboration with colleagues, supervisors, clinical staff, community members, and families. DSPs are typically assigned to residential homes involving more complex and difficult cases including, but not limited to, medically frail or elderly residents, individuals with acquired brain injury, individuals with developmental or intellectual disability, or individuals with other related clinical or behavioral diagnoses.

Flexible schedule and reimbursed mileage for transportation.

High school diploma or equivalent required. Associates degree or additional education in the field (s) of psychology, social work, nursing, or special education preferred.  1 year of direct care, CNA, or human services experience preferred. Valid driver’s license in the state of Massachusetts required.

Community Health Nurse

As a member of the nursing and care management team, provides nursing services to facilitate the screening of new foster homes and participants; assists with matching and placement services and maintains services to participants, as needed.

Registered Nurse licensed by the Massachusetts Board of Registration in Nursing. Minimum 2 years prior nursing experience in the direct care of the elderly or disabled adults. Valid driver’s license in the state of Massachusetts required.

Individual Supports Case Manager

Full Time

Provides instruction, supports and positive role modeling to individuals with varying developmental disabilities and/or acquired brain injury living in their own homes.  Encourages the development of life, social, and recreational skills, as well as community access and healthcare.  Provides direct, active and constructive interactions with individuals in cooperation with staff, team members, clinical consultants, community members, families and managers.

Associates Degree or Bachelor’s Degree preferred. Education in psychology, social work, special education, or other related field preferred. Prior human services or relevant life experience preferred of at least 2 years. Valid driver’s license in the state of Massachusetts.

Residential Program Manager

Full Time

In partnership with residential management, implements a comprehensive system of highly personalized residential supports which create a safe and therapeutic environment for program participants Bachelor’s degree preferred.
Minimum 6 years prior experience including 3 years residential management and staff management experience and 3 years supporting individuals with intellectual and developmental disabilities, autism, brain injuries or other unique challenges.
Knowledge of regulatory compliance. Prior supervisory experience required. Valid driver’s license in the state of Massachusetts required.

Caregiver

We are seeking caring and dedicated people to join our Shared Living team by becoming caregivers. Caregivers open their homes and lives to a person with disabilities while supporting their independence and self-determination. Our seasoned professional team will work diligently to pair you with a suitable individual and to create a meaningful and purpose filled life for all.

Caregivers are considered subcontractors who are given a monthly, tax exempt stipend. You will also receive supports from nursing, case management, and clinical professionals, as well as thorough and ongoing trainings.

Caregiver qualifications: Valid driver’s license and own vehicle. Extra bedroom in their rented or owned house. Must be found suitable through a CORI and fingerprint check.

Community-Based Day Supports (CBDS)

Community Resource Specialist

Full Time

The Community Resource Specialist provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. Provides support and assistant to program participants in the areas of pre-vocational/job preparation, socialization, community orientation/education, and wellness.

Developmental Specialist II

Assists individuals in the areas of independent living, self-management within the community, social and independent living skills development and affective and behavioral management. Assists in development and implementation of behavioral plans and provides behavioral management and intervention as required.

Bachelor’s degree or additional education in the field (s) of psychology, social work, nursing, or special education required. Master’s degree preferred and evidence of ABA professional training. Minimum of 4 years prior experience in behavior intervention.Valid driver’s license in the state of Massachusetts required.

Vocational Specialist

The Vocational Specialist works with program participants from referrals made through State Agency contracts and is responsible for completing vocational assessments and assisting individuals to secure competitive community employment by providing and arranging the necessary job supports to ensure success. Social Enterprise

Lead Grower, Farm

Full Time

The Lead Grower is responsible for the day to day crop management at Cape Abilities Farm in Dennis. Working closely with the Cape Abilities Farm Director, the Lead Grower will oversee, and work alongside production crews to produce top-quality ornamental and edible crops. Bachelor’s or Associate’s degree in agriculture related field required, and/or 3 years prior greenhouse/agriculture experience. Valid driver’s license in the state of Massachusetts required.
