The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access.

High school diploma or equivalent required. 1 year of direct care, CNA, or human services experience preferred. Ability and willingness to provide hands-on personal care. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required.

DSPs are typically assigned to residential homes involving more complex and difficult cases including, but not limited to, medically frail or elderly residents, individuals with acquired brain injury, individuals with developmental or intellectual disability, or individuals with other related clinical or behavioral diagnoses.

Residential LPN

Full Time & Part Time

Provides episodic nursing care and assistance with other residential tasks, as needed. Consults with residential staff regarding health issues of individuals in accordance with the Department of Developmental Services (DDS), Medication Administration Program (MAP) and Agency policies. Provides direct oversight and training of staff on shift in meeting individual needs. 

Licensed Practical Nurse (L.P.N.). Obtains and maintains MAP Certification. Minimum 2 years prior experience in community health nursing, visiting nursing and/or nursing specific to individuals with unique developmental or traumatic brain injury challenges. Valid driver’s license in the state of Massachusetts. Successful completion of all state and Agency mandated trainings. Ability to provide hands-on personal care.

 Assistant Residential Program Director

 Full Time – Hyannis

The Assistant Residential Program Director is responsible for the successful daily operations of our comprehensive Residential Supports creating a safe and therapeutic environment for the people served. The Assistant Program Director will focus on coordinating the onboarding and supervision of a Residential Management Team as well as a DSP team.

Bachelor’s degree preferred. Minimum 6 years prior experience including 3 years residential management and staff management experience and 3 years supporting individuals with intellectual and developmental disabilities, autism, brain injuries or other unique challenges. Knowledge of regulatory compliance. Prior supervisory experience required. Ability and willingness to provide hands-on personal care. Strong organizational skills. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required. Successful completion of all state and Agency mandated trainings required upon hire. Successful completion of Medication Administration Training certification required upon hire.

Caregiver

We are looking for a compassionate caregiver family or a couple in Mid-Cape area. Candidates must undergo background check, have a spare room at ground level in their home, reliable vehicle and make a difference in other persons life. Caregivers will be provided with administrative support and training with a generous tax free stipend.

The Lead Grower is responsible for the day to day crop management at Cape Abilities Farm in Dennis. Working closely with the Cape Abilities Farm Director, the Lead Grower will oversee, and work alongside production crews to produce top-quality ornamental and edible crops. Bachelor’s or Associate’s degree in agriculture related field required, and/or 3 years prior greenhouse/agriculture experience. Valid driver’s license in the state of Massachusetts required.

The Thrift Truck Driver picks up donations at donors’ locations, and delivers purchases to customers. The Thrift Truck Driver also helps process drop-off donations, helps load customer vehicles, and provides top-class customer service.

CBDS Coordinator

Full Time 40 hours

Assist in the management of the Community Base Day Support (CBDS) Program, ensuring performance and effectiveness. Provides leadership for Community Resource Specialists in the creation of goals and objectives in Individual Service Plan (ISP) format. Assists individuals with making and expressing choices based on assessments.  Plans program activities and supervises implementation.

Bachelor’s degree in human services or related field. Master’s degree preferred.  Minimum of 5 years prior human services experience.  Ability to model and teach occupational tasks. Attention to detail. Valid driver’s license in the state of Massachusetts.
